Waiver and consent

Read it once. Each box is its own agreement.

Read the waiver and privacy notice

Sport carries risk. Basketball involves running, jumping and contact. Injuries can happen even when everything is done right: sprains, fractures, cuts, concussion, and in rare cases worse. By registering you accept those risks for the player.
Fit to play. You confirm the player is fit to train and play and that the health notes list any condition we should know about.
Emergency treatment. If the player is hurt and cannot consent, our staff may give first aid and, if needed, take the player to a hospital or clinic. We call the contact on this form right away.
What this waiver does not do. It does not excuse F.A.E. from its own negligence or from keeping the venue reasonably safe. Your right to make a claim about those stays.
Your data. F.A.E. Sports Management Services, Lipa City keeps these details in the FAE ACADEMY records to run the program and reach you in an emergency (Data Privacy Act of 2012). We do not sell them or share them with advertisers. Records are deleted 2 years after the player's last season. You can see, correct or delete your record any time by messaging us.
How it is sent. When you tap Submit, your answers go to F.A.E.'s private online records. The public cannot read them. Your QR receipt holds what you typed, so share it only with F.A.E.
Photos and video. Only if you tick the photo box below. We never post a child's full name, school or contact details with their image, and you can change your mind any time.
