  • Sport carries risk. Basketball and volleyball involve running, jumping and contact. Injuries can happen even when everything is done right: sprains, fractures, cuts, concussion, and in rare cases worse. By registering you accept those risks for the player.
  • Fit to play. You confirm the player is fit to take part and has told us about any condition in the health notes.
  • Emergency treatment. If the player is hurt and cannot consent, our staff may arrange first aid and, if needed, transport to a hospital or clinic. We will call the contact above right away.
  • What this waiver does not do. It does not excuse F.A.E. from its own negligence or from keeping the venue reasonably safe. Your right to make a claim about those stays.
  • Your data. F.A.E. Sports Management Services, Lipa City keeps these details to run the program and reach you in an emergency. We do not sell them or share them with advertisers. Records are deleted 2 years after the player's last season. You can see, correct or delete your record any time by messaging us.
  • How it's sent. When you tap Create my QR, your answers go straight to F.A.E.'s private online records, kept with our cloud provider, where the public cannot read them. We move them into our office records and clear the online copy within 30 days. Your QR receipt holds what you typed, so share it only with F.A.E.
  • Photos and video. F.A.E. photographs and films training, games and events, and may post them on our pages and social media. Accepting this waiver includes that. We never post a child's full name, school or contact details with their image. Rather the player is not posted? Message us any time and we leave them out and take down what we still control.
